CMSFdt Updates

I added a "published date" to the documents, that gets updated when the document is (as the name imply) published and is cleared when it is un-published. This allow me to see which documents have been published and when. Before I only had the editing date that was when a document was modified in some way, published or not.

The code changes are published in Git and the database creation script is updated, also the 'update' script for the database is also updated.

Comments are added when and more important if I have the time to review them and after removing Spam, Crap, Phishing and the like. So don't hold your breath. And if your comment doesn't appear, is probably becuase it wasn't worth it.
